Massage Terms

Initial appointment time includes a discussion to obtain health history, informed consent, physical assessment, time to remove and put clothing back on, time to get on and off the table, explanation of remedial exercises and hands on treatment. You understand that during subsequent visits, any assessments performed by the massage practitioner are part of the appointment time.

If you experience pain or discomfort during the session, you will immediately inform the Massage Therapist so that pressure/technique can be adjusted to your level of comfort. You will not hold the Massage Therapist responsible for any unwanted pain or discomfort you experience during or after the session.

You understand that the services offered today are not a substitute for medical care. You understand that the Massage Therapist is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness.

You affirm that you have notified the Massage Therapist of all known medical conditions and injuries.

You agree to inform the Massage Therapist of any changes in your health and medical condition. You understand that there shall be no liability on the massage therapist’s part should you forget to do so.

You understand that massage is non-sexual in nature.

You agree to pay the agreed fee before or immediately after each individual visit, and that any discount will be at the discretion of the Massage Therapist and will be recorded your my file.

By agreeing to these statements, you hereby waive and release the Massage Therapist from any and all liability, past, present, and future relating to the practice of massage and bodywork, and that you undertake massage from the massage practitioner at your own risk.

Privacy Policy

Your privacy is important to us. The purpose of this policy is to tell you what personal information is collected, how we use it, whether it is disclosed it to anybody else, how long it is kept and how you can access your personal information.

Personal information: Personal information means any information about you except your name, home address and telephone number if these are published in a telephone book or other directory. Business information (i.e. business cards) is not considered personal.

Accountability: My massage practice is a sole proprietorship; therefore I am responsible for the personal information under my control and am also the designated Privacy Officer for my business. What personal information do we collect? When you receive a service from the clinic, we may collect some or all of the following personal information:

1. Name, address and telephone number

2. E-mail address and facsimile number (if applicable)

3. Occupation

4. Date of birth

5. Next of kin and emergency contact information

6. Similar information about other members of your family

7. Health information

The security of your personal information is important to us: we use your personal information for the purposes identified below. Files are kept in a locked area. In extremely rare cases, your records may be subpoenaed by the legal system. In this instance, we are obligated to turn over copies of your records to the responsible authorities.

How do we use your personal information? Your personal information is used by us to determine if massage or other services available to you could be of benefit to you. It is also to ensure that you get the best possible care for any and all of your health concerns.

We do not sell your personal information to third parties. We may share some or all of your information with other health care providers you are already affiliated with, ONLY with your written permission. Information may be shared with an insurance company, lawyer or otherwise with written permission.

How long do we keep your personal information? We retain your personal information for a ten (10) year period after your final visit, which is mandated by our governing body.

You can request access to your personal information: You have a right to access the personal information that we have about you. You also have a right to request the personal information, which you believe to be inaccurate, be corrected. If you wish to access your personal information you should contact us. A fee may be charged in instances where an office visit is needed to go over a file, or if a photocopy of a file is requested.
